Reimagining palliative care delivery with Home Nursing Foundation to improve visibility, reduce delays, and support better care beyond home visits.
Home Nursing Foundation (HNF) supports over 1,000 patients each year, many with complex or end-of-life care needs, through its Integrated Home Health (IHH) programme. With a lean team of doctors and nurses managing large patient loads, delivering timely and personalised care between home visits can be challenging.In partnership with HNF, Speedback is exploring how redesigned care workflows and digital communication tools can improve visibility into patients’ conditions between visits. By enabling patients and caregivers to share updates more easily, the goal is to support earlier interventions, strengthen care coordination, and reduce the operational burden on care teams while maintaining compassionate, patient-centred care at home.
